Our staff will gladly verify your Physical Therapy insurance benefit and answer any questions you may have. We want to ensure our patients have a solid understanding of the insurance plan.

Your Insurance Verification

Core Physical Therapy will verify your insurance information for Physical Therapy services at/prior to your arrival. We will provide you with your plan details, including deductibles, co-pays, co-insurance responsibility, and any limitations to your benefit. We will also provide you with an estimated cost per visit. (Verification of benefits does not guarantee payments of claims.)


Our Orthopedic Physical Therapists are in network with:

  • Aetna – PPO, POS, and HMO Plans
  • Blue Cross Blue Shield of Illinois – all PPOs, Excluding HMOs
  • CIGNA Plans
  • Humana ChoiceCare – PPO plans, HMO, POS, Medicare
  • Medicare
  • Third-party Insurance, Motor Vehicle Accidents
  • TriCare Plans
  • United Healthcare – Choice PPO plans, Medicare Advantage, and Harvard Pilgrim Plans
  • Workers’ Compensation Claims

Please note: Starting January 1st, 2022 our Pelvic Health Physical Therapists will only be in-network with:

  • Blue Cross Blue Shield of Illinois – all PPOs, Excluding HMOs
  • Medicare
  • Third-party Insurance, Motor Vehicle Accidents
  • Workers’ Compensation Claims

Patients in the Pelvic Health Program with the above insurances will have claims submitted to their insurance carrier on their behalf.  Patients with other insurance plans will be billed $120 per visit at the time of service.  Core Physical Therapy will provide the necessary paperwork to submit claims to their carrier for reimbursement.

Why are the Pelvic Health Physical Therapists out-of-network with other plans?

Unfortunately, being a participating provider in these plans has become significantly harder due to their continued decrease in reimbursement rates.  Core Physical Therapy has attempted numerous times to address our concerns with these carriers and emphasized that our Pelvic Health program requires a higher level of skill and time and should merit a reasonable reimbursement rate.  These insurance companies have declined to engage in a meaningful discussion.  Core PT has always maintained a high standard of care for our patients, and as a company, we are unwilling to compromise or cut corners due to insurance company shortcomings.

There are many benefits of being out-of-network with these plans for our Pelvic Health patients.  Our therapists will be able to spend a full hour every visit with their patients for a more comprehensive treatment.  This will hopefully allow for fewer overall visits.  Additionally, these other carriers frequently require pre-authorization for care to be rendered.  This means that patients are given a certain amount of visits that must be used in a specific timeframe.  We all know that life doesn’t necessarily work life that.  Being out-of-network will allow patients to be seen when they are in need and as often as they feel they need to reach their goals.
